Why Swimming Is a Great Full-Body Workout for Amputees

    Swimming is one of the best exercises that almost anyone can do. This is because water changes the rules of exercise: buoyancy takes weight off the body, resistance builds strength without heavy equipment, and neither depends on how many limbs are doing the work. That combination is part of why swimming is so often recommended for the limb loss and limb difference community.

    Water Takes the Weight-Bearing Load Off the Body  

    On land, a missing limb often shifts the workload to other parts of the body. Below-knee and above-knee amputees tend to depend more on their sound-side leg for weight-bearing and balance, while upper-limb amputees modify their posture and grip to compensate. Over time, this asymmetrical load can cause extra stress on the joints that bear the weight.

    Water changes this dynamic. Buoyancy supports the body, reducing joint strain during exercise and making activities like walking, running, or cycling much easier. Although water provides resistance, it remains low impact. For individuals whose sound-side hip, knee, or shoulder already bears extra weight, this is significant: swimming is one of the few exercises that doesn’t add load.

    That’s why swimming is often recommended for arthritis and mobility issues linked to limb loss:

    • Arthritis: Swimming helps ease symptoms and boosts muscle strength.

    • Reduced mobility: Water exercises can lessen pain and spasticity for those with mobility impairments.

    A Full-Body Workout, Even for Limited Limb Function  

    Water offers greater resistance than air, making each stroke and kick require more muscle effort than land movements. This resistance helps build strength and increases calorie burn. Unlike typical gym equipment or bodyweight exercises, swimming doesn’t target a fixed set of limbs.

    So, a person weighing 154 pounds swimming slow freestyle laps for 30 minutes burns about 255 calories, though this varies depending on weight, speed, and stroke.

    Swimming Without a Prosthesis: What to Plan For  

    Adaptive swimming requires some practical planning. Here are some things you need to consider before getting started: 

    • Most amputees swim without their everyday prosthesis, as standard prosthetic devices aren’t designed for water or chlorine exposure. While some manufacturers offer water-resistant or swim-specific prosthetic legs for pool and open-water swimming, many swimmers choose to go without a prosthesis, which still allows for a complete full-body workout.

    • Residual limb skin care after swimming. Chlorine and extended water exposure can dry out or irritate skin. So it’s crucial to rinse and thoroughly dry your residual limb before putting your prosthesis back on. This helps prevent irritation or skin breakdown.

    • Getting in and out of the pool. Without a prosthesis, planning your pool entry and exit is important. Options like transfer benches, pool lifts, or steps and ramps can help you enter and exit safely.

    • Balance takes some relearning. It’s normal to experience some balance adjustments when swimming without a limb.

    • Adaptive and para-swimming lessons. Instructors trained in adaptive swim technique can help swimmers (including swimmers relearning technique after amputation) find a stroke that works for their bodies.

    Benefits for Heart, Lungs, and Mental Health  

    Regular swimming has been linked with better cholesterol levels, lower blood pressure, and a decreased risk of heart disease. And research backs this. Individuals who swim are 41% less likely to die from heart-related illnesses or strokes and have a 28% lower risk of premature death in comparison to those who do not swim, according to a study sponsored by Swim England. It also enhances lung strength and capacity, making it recommended for individuals with asthma or COPD.

    But those with heart conditions or recovering from surgery, including recent amputations, should seek medical advice before swimming.

    On the mental health side, a study on women with fibromyalgia found that eight months of warm water swimming yielded a 41% reduction in anxiety and a 27% reduction in depression, along with physical benefits. Many swimmers also find the activity meditative.

    Getting Started  

    Start small. Even swimming the length of the pool can be harder than expected at first. Build up gradually—five minutes, then 10—rather than expecting to swim for half an hour right away.

    Stick to pools at first. Learn technique in a controlled pool environment before moving into open water like lakes or bays.

    Look for a lifeguard. A guarded pool means trained help is on hand if something goes wrong.

    Stop if it hurts. Pain—especially in the shoulders, hips, or residual limb—is a signal to stop and check in with a healthcare provider.

    Loop in a prosthetist or physical therapist, not just a doctor. Along with general medical clearance, it’s best to ask your prosthetist or physical therapist whether there’s anything specific to your residual limb or amputation level to watch for in the water.

     

    Any exercise is better than none, and swimming doesn’t require a specific time or distance target before it starts delivering benefits. Whether it’s a few laps, water walking, or an adaptive swim class, swimming is one of the few workouts that builds strength and protects the joints without asking the body to be anything other than what it is.
